Toe Walking SMO3D
The Toe Walking SMO3D enables children with mild to moderate toe walking to achieve better foot and ankle alignment and a more efficient heel-toe gait. Its extended posterior tongue offers a consistent physical reminder for heel contact, reducing excessive plantarflexion while still supporting natural movement. Trimlines promote a propulsive gait, letting kids develop improved balance, energy efficiency, and lateral weight shifts, thereby actively supporting gross motor progress.

Indications:
- Toe walking due to sensory or stability concerns
- Triplanar instability in weight bearing
- Excessive pronation
- Sensory deficits
- Ability to get foot 90 degrees with knees extended
- More than 50% of the time spent on toes (unchanged with shoes)
- Anterior weight lines and lack of lateral weight shifts
Contraindications:
- Requires more sagittal plane control
- Requires more knee control
- Will need plastic modifications to address severe toe walking
- Heavier body weight or very bony foot (Big Shot SMO would be indicated)
- Will need plastic modifications to control adduction instead of abduction
- Rigid deformity
- Severe spasticity
- Plantarflexion contractures
- Requires full footplate
- Cannot get foot to 90 degrees with knees extended

Dynamic Stability:
These devices provide stability while allowing for flexibility/movement and address atypical tone. Patients are able to actively correct when cued.
Currently, the Toe Walking SMO3D is a more standardized device due to the manufacturing process. If customization is required beyond what we offer in the ordering portal, we recommend exploring thermoplastic options like the Surestep Toe Walking SMO.
